NOTICE OF DATA BREACH

A data breach occurred at Cochise Eye and Laser, on January 13th, 2021. We were attacked by a ransomware virus. This incident involved the encryption of our patient scheduling and billing software. There is no evidence that the data was taken, only that it was encrypted, and in some cases deleted, making it impossible for us to access anything in our scheduling system. Our office is still operating with paper charts, so we can continue care of our patients. We have been working on implementing increased security measures, recovering data, and a new offsite backup. We will be using charts to rebuild our schedules. Everyone seen after January 1st, 2020 will be called to reschedule follow up appointments, as we have no way of knowing when they were originally scheduled. Although there is no evidence the data was taken, this is considered a breach of protected health information. Names, dates of birth, addresses, phone numbers, and in some cases social security numbers were stored in our billing software.
What We Recommend:
We recommend that our patients place a fraud alert on their credit file. A fraud alert tells creditors to contact you before they open any new accounts or change your existing accounts. Call any one of the three major credit bureaus listed below. As soon as one credit bureau confirms your fraud alert, the others are notified to place fraud alerts. The initial fraud alert stays on your credit report for one year. You can renew it after one year.

You may also request that all three credit reports be sent to you, free of charge, for your review. Even if you do not find any suspicious activity on your initial credit reports, the Federal Trade Commission (FTC) recommends that you check your credit reports periodically. Checking your credit reports periodically can help you spot problems and address them quickly. If your personal information has been misused, visit the FTC’s site at IdentityTheft.gov to get recovery steps and to file an identity theft complaint. Your complaint will be added to the FTC’s Consumer Sentinel Network, where it will be accessible to law enforcers for their investigations.
